The rise in chronic illnesses in an ageing Singaporean population has sparked a strategic policy shift towards preventative healthcare. This new care model aims to nudge individuals to take ownership of their general health by following a personalised health plan charted out by their family clinician.
After each clinician consultation, the plan is updated in the national healthcare app, allowing individuals to track their progress towards a healthier lifestyle.
Feedback on the previous version of the app revealed that Singaporean residents’ current health plans are too generic and do not encompass a holistic view of personal health.
PSYKHE partnered with key governmental agencies to deliver a new personalised health plan. This plan aims to cultivate preventative healthcare habits in 3 key areas: vaccination adoption, diet and exercise, and smoking cessation.
Over 16 weeks, PSYKHE adopted an iterative design, test and refine methodology. 3 principles underlined our approach:
Our approach followed 4 design sprints across 3 key stages from discovery to delivery.
Stage 1: Discovery & Ideation
To envision our re-imagined health plan, we analysed prior qualitative research and industry best practices as a starting point for new ideas.
Understanding key issues with the legacy app allowed us to facilitate the co-creation of concept screens with clinicians and healthcare agency stakeholders. Through these sessions, we fostered a sense of ownership and stakeholder buy-in over the designs.
Stage 2: Concept Generation & Validation
With the concepts in place, we refined them through resident and clinician testing.
The insights from our concept tests generated further design iterations. At the end of each sprint, we turned these iterations into high-fidelity screens.
Stage 3: Consolidation & Delivery
PSYKHE consolidated our designs into a seamless mobile experience that integrated well with existing BAU features.
Finally, by testing the prototype with 35 Singaporean residents, we validated that initial user needs were met through effective design.

PSYKHE’s emphasis on our 3 governing principles enabled:
Through the re-designed national health plan, residents are nudged towards a self-motivated, preventative care approach that will reduce reliance on healthcare services in the long term.
